Declaration & liability release

Please read carefully, then confirm all three statements.

Assumption of Inherent Risk. I voluntarily choose to participate in the selected adventure activity conducted by Greeny Adventures and Resorts. I acknowledge that adventure activities carry inherent risks which cannot be eliminated even with reasonable care, including slips, falls, jolts, collisions, physical strain and, in rare cases, serious injury. I voluntarily assume these inherent risks.

Limited Release. I agree that the Corporation and its operators shall not be liable for injury, loss or damage arising from: (a) risks inherent to the activity; (b) my failure to follow staff instructions or safety guidelines; (c) any medical condition or physical limitation I have not disclosed; or (d) my participation while under the influence of alcohol or drugs.

I am physically and mentally fit, have no medical condition or physical limitation (including pregnancy) affecting safe participation, and am not under the influence of alcohol or drugs.

I will follow all instructions, rules and safety guidelines given by staff, and will use all safety equipment provided.

I have read and understood the Assumption of Inherent Risk and Limited Release above, and voluntarily assume the inherent risks of the activity.

I confirm I am the parent/legal guardian of the participant and am present at the venue. I confirm the participant is medically fit for the selected activities and I have disclosed all relevant medical conditions. I authorise their participation. I personally indemnify the Corporation and its operators against any claim arising from information I have withheld or misstated, or from the participant's failure to follow safety instructions. I understand this does not affect the participant's own legal rights.
